Pre-Authorized Healthcare Forms (PAHCFs) are generated when you store a patient's card on file for future payments in the Vault or set up a Recurring payment plan. PAHCFs in Bridge Payments are used to capture the patient's consent and may help protect against chargebacks. These forms capture information electronically and are kept on file for future reference.

PAHCFs can include the following information:

  • Details of the recurring payment plan.
  • The maximum amount to be charged and whether the maximum amount applies to All visits this year or This visit only for cards on file in Vault.
  • The cardholder’s signature.
    Cardholder signatures can be captured digitally if Bridge Payments is connected to a card reader, or you can obtain their signature on a printed copy of the PAHCF for your records if no card reader is connected.

You can view, email, or print the PAHCF the moment it's generated and afterwards.
